29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On January 25, 2012 employees were not protected by hard hats while working around loads supported by a crane, exposing them to a struck-by hazards. In accordance with 29 CFR 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On January 25, 2012 employees were not protected from a fall while walking/ working from the roof of a commercial building, exposing them to a fall hazard of 15 feet. In accordance with 29 CFR 1903.19(d) abatement documentation is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
