29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation. a. At the jobsite, Pump Jack scaffold legs were placed on loose gravel and pieces of concrete scrap without base plates or mud sills, therefore exposing employees to a fall hazard.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of thissection, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4)of this section. A. At the job site, employees were working on a Pump Jack scaffold 10' 5" high without the body harness lanyard attached to the rope grab attachment device thereby exposing employees to a fall hazard.
