29 CFR 1926.451(c)(1)(ii): Guys, ties, and braces shall be installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and be repeated vertically at locations of horizontal members every 20 feet (6.1m) or less there after for scaffolds 3 feet (0.91 m) wide or less, and every 26 feet (7.9 m) or less there after for scaffolds greater than 3 feet (0.91m) wide. The top guy, tie or brace of completed scaffolds shall be placed no further than the 4:1 height from the top. Such guys, ties and braces shall be installed at each end of the scaffold and at horizontal intervals not to exceed 30 feet (9.1 m) (measured from one end [not both] towards the other). At the worksite, the employer did not ensure/enforce that the 105 foot long, 5 to 6 buck high tubular welded scaffold was tied and braced at intervals not to exceed 30 feet, thereby exposing employees to approximate 36 foot fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs,and other elevated surfaces are set forth in subpart M of this part. At the worksite, the employer did not ensure/enforce that the tubular welded scaffold was guarded by the stack of block that was being transferred by an employee to the block layers that were building the NE wall of the gymnasium, thereby exposing the employee to approximate 36 foot fall hazard.
