The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) Worksite, Bindery area: The employer did not develop or implement a Lockout/Tagout program for employees whose duties include removing guard to clear paper jam in industrial equipment such as Omega 2 - Binder Machine; on or about 1/25/12. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
