Lewis Machine & Tool Co., Milan, IL:  On or about January 27th, 2012, the employer did not provide the required injury or illness descriptions on the OSHA 300 Log for the calendar year 2009.    a)  On or about January 5th, 2009, an employee received a recordable illness.
