Hardservice flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usagecharacteristics of the cord being spliced.    a.  On or about January 26, 2012 - at the above addressed jobsite, a flexible cord with visible splices was used to power electrical tools such as a chop saw and a battery charger, thereby exposing the employees to hazards associated with electrical shock.

Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safetynet system, or a personal fall arrest system.    a.  On or about January 26, 2012 - at the above addressed jobsite, employees conducting window frame installation were exposed to falls of approximately nineteen (19) feet, four (4) inches created by the presence of a wall opening.
