29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: Employees repairing a broken water line were exposed to crushing and asphyxiation hazards. The excavation measured approximately 6.5 feet to the floor, the excavation walls were not shored, sloped or benched, the spoil pile was approximately 2 inches from the mouth of the excavation, and the soil composition had not been determined. The excavation and adjacent areas were not inspected by a competent person.
