29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a) On or about January 25, 2012, and at times prior, the employer did not ensure the extension cords used to power carpentry tools were safe in that the insulation on one of the cords was cut and covered with electrical tape rather than properly repaired or removed from service.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. a) On or about January 25, 2012, and at times prior, Tafoya Construction, Inc., as the creating and exposing employer, did not install midrails on a guardrail system above or on the stairway from the basement to the main floor at a house under construction at 6156 E. Powers in Greenwood Village, Colorado.
