(a) At FSD Bldg 402, East end, wood shop power panel A, circuit breaker #19 was taped and breakers #12 and 14 were tied together.

29 CFR 1910.303(b)(2):   Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.      (a) At the Air Clearance Authority room, a relocatable power tap (RPT) was used to plug-in 2 Mr coffee, coffee maker and a refrigerator.      (b) At the Assistant Transportation office, 2 relocatable power taps were daisy chained, one RPT was connected to the other.      (c) At the Supply Clerk's room, a relocatable power tap was used to plug-in a microwave oven, oscillating fan, Keurig coffee pot and a printer.      (d) At the QC/QA break room, a relocatable power tap was used to plug-in a Hamilton Beach Coffee maker and a toaster oven.      (e) At employee break room of Bldg 560, 2 relocatable power taps were daisy chained, one RPT was connected to the other.

(a) At Bldg 402, the swamp cooler power cord and power cord in the scale area, did not have strain relief, insulation was separated from the plug.
