29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:    (a) D & L Construction on or about January 27, 2012 at the address of 18018 Tontogany Creek Rd. Tontogany, OH: The employer failed to ensure a safe means of access/egress was provided to employees. During steel erecting work, employees were utilizing a Genie model #GS-2668 RT scissor lift to gain access to an elevated work area. Employees were required to step more than 2ft horizontal and 4ft vertical from the scaffolding platform to access the working area. These employees were exposed to a 14ft fall to the ground below.
