29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: At Ford Island, building 26, an extension cord used to supply power to a DeWalt chop saw, serial number 666885, was plugged into a 120 VAC recepticle (PNL 112 CKT 3), having reverse polarity. Employees were exposed to shock hazards.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. (a) At Ford Island, outside building 26, an operational ground fault circuit interrupter or an assured equipment grounding conductor program was not used, when employees were using the 110 VAC DeWalt chop saw. Employees were exposed to the hazards of contacting electricity.
