29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: On or about January 23, 2012 and at times prior thereto, employees were exposed to the hazard of not being able to locate the exit door in the event of an emergency. The electric supply cord to the exit light was not plugged into an electrical outlet.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were attached to building surfaces. On or about January 23, 2012 and at times prior thereto, employees were exposed to the hazard of electrical shock due to the flexible cord of the exit sign securely fastened to the wall in place of permanent wiring.
