29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):    (a)  The employer did not ensure that the rough terrain forklift operator was properly trained and certified to operate the forklift prior to being allowed to utilize the CAT (Model: TH63) rough terrain forklift to lift sheathing material.  This exposed employees to crushing and struck-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or perso  (a)  At 12400 North Ville du Parc Drive in Mequon, Wisconsin, employees were conducting sheathing work on a commercial building.  The employer failed to provide employees with fall protection, exposing employees to a fall hazard of approximately 24.4 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  The employer failed to provide a training program for each employee conducting sheathing installation on the roof of a commercial building at 12400 Ville du Parc Drive in Mequon, Wisconsin.  The employees were not provided with training on identifying fall hazards, and procedures to eliminate or minimize the hazard.  This exposed employees to fall hazards.
