29 CFR 1910.95(g)(6):     At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :    a) Line 515, on or about 1/31/12:  A Line 515 assistant was exposed to a 138.3 % noise dose, or 92.35 decibels (dBA), during the 334 minute sampling period. A zero increment was included for the 146 minutes not sampled. Two employees working at this position were not given an annual audiogram for the 2011 calendar year.    b) Line 518, on or about 1/31/12:  A Line 518 assistant was exposed to a 138.3 % noise dose, or 92.35 decibels (dBA), during the 334 minute sampling period. A zero increment was included for the 146 minutes not sampled. An employee working at this position was not given an annual audiogram for the 2011 calendar year.
