29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a)A relocatable power tap was plugged into an extension cord in the 2nd to last outer office Northwest side.   The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical  fire hazards.      (b)  A Warm Fusion Heater was plugged into a relocatable power tap in the cubicle center East (3rd from East wall).   The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.      The Food and Drug Administration was previously cited for a violation of this standard which was contained in OSHA inspection no. 309098564 citation 1, item 2, issued on December 2, 2010 with respect to a workplace at Blaine, WA.    The Food and Drug Administration was previously cited for a violation of this standard which was contained in OSHA inspection no. 309094720 citation 2, item 1, issued on November 21, 2008 with respect to a workplace at Tacoma, WA.
