Each such employee shall be trained to know at least the following:    The employer is failing to provide employees with training on using personnel protective equipment for protection against laceration hazards.  This was most recently evident during the inspection of the facility located at 129 West Church St. Albion, NE 68620, where the employer has untrained employees butchering cuts of beef using knives without wearing cut resistance gloves to protect their non-cutting hands.

29 CFR 1910.212(a)(3)(iii):     Special handtools for placing and removing material shall be such as to permit easy handling ofmaterial without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but canonly be used to supplement protection provided.    The employer is failing to protect employees from potential amputation hazards associated with cutting small product on a vertical band saw without using the necessary hand tool.  This was most recently evident during the inspection of the facility located at 129 West Church St. Albion, NE 68620, where the employer had an employee using the Butcher Boy vertical meat cutting band saw to cut ? hamburger patties out of a 5lb tubes; when getting within 5 of the cutting surface the employee failed to use the pusher tool to maintain a safe distance from the cutting portion of the blade thus resulting in the employee being injured and having a finger partially amputated.

29 CFR 1910.1030(g)(2)(i):     The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program.    The employer is failing to provide employees with blood borne pathogen training.  This was most recently evident during the inspection of the facility located at 129 West Church St. Albion, NE 68620, where the employer has employees cutting meat product using tools such as saws and knives thus exposing employees to potential blood borne pathogen hazards in the event of an employee cutting themselves without having provided any training on how to handle the exposure and prevent the employee from contacting a blood borne disease.

29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals intheir work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trainedabout is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability,carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety datasheets.    The employer is failing to provide employees with HAZCOM training.  This was most recently evident during the inspection of the facility located at 129 West Church St. Albion, NE 68620, where the employer has untrained employees using various chemicals to clean the facility on a daily basis.  Instances include but are not limited to:    i. Employees mixing Clorox bleach with water to use in cleaning the facility without management specifying the ratio.    ii. Employees unfamiliar with material safety data sheets, specifically for Clorox bleach; Clorox bleach contains Sodium hypochlorite.

29 CFR 1904.29(b)(3):     How quickly must each injury or illness be recorded? You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.    The employer failed to enter a recordable injury on the OSHA 300 log and to fill out a 301 Incident Report within seven (7) days of an accident occurring.  This was most recently evident during the inspection of the facility located at 129 West Church St. Albion, NE 68620, where the employer had an employee injured while using a band saw thus amputating a portion of her finger and thumb; without annotating this accident in an Incident Report or updating the OSHA 300 log.
