29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:   (a) Employees conduction siding work on pump jack scaffolding systems that were not provided with base plates, mud sills or other adequate firm foundations for the support poles.   (b) Employees were exposed to fall hazards while climbing on and off the pump jack scaffolds without appropriate ladders.

29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   (a) Employees were not properly trained in the recognition and avoidance of fall hazards when working from the roof of a residential building.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    (a) Employees were conducting siding work on a pump jack scaffolding system. Both pump jack scaffolding systems on site did not have base plates or mud sills or adequate firm foundation for each of the poles, exposing employees to fall and struck by hazards if the scaffolding system failed.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      (a)	The scaffold platform was greater than 2 feet below the roof and an employee climbed on the roof from the scaffold without sufficient means of access on to the roof exposing the employee to a fall hazard of approximately 10 feet. This exposure was above 7313 Beloit Rd, West Allis WI 53219.    (b)	An employee climbed down off a scaffold and used the hand rail of a porch to get to the ground. The access was greater than 2 feet below the scaffolding. This exposure was in front of 7313 Beloit Rd, West Allis WI 53219.

(a) Employees were conducting roofing work without any fall protection system in place above 7313 Beloit Rd, West Allis WI 53219. The employees were exposed to a fall hazard of approximately 10 feet to ground.    (b) Employees were conducting roofing work without any fall protection system in place above 7325 Beloit Rd, West Allis WI 53219. The employees were exposed to a fall hazard of approximately 10 feet to ground.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) The employer did not ensure that employees were using fall protection when conducting work on the roof while being exposed to a fall hazard. Employees were exposed to a fall hazard of approximately 10 feet. The employer did not ensure that the employees were trained to recognize and protect themselves from fall hazards.
