29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On 1/30/2012, the following machines in the tool room were not properly safeguarded.    a) The two Bridgeport milling machines did not have rotating chuck guarding.  b) The Carlton and the Hercules radial arm drill presses did not have rotating chuck guarding.   c) The DoAll horizontal bandsaw had 3" of the used portion of the saw blade under the platen that was not guarded.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:      On 1/30/2012, at the Verson 21 press there was an unsecured electrical receptacle junction box with knockouts, attached to an electrical flexible cord.     NOTE: Knockout boxes were designed to be secured to a fixed/permanent structure. When knockout boxes are used on the ends of flexible cords, employees gripping the box to attach or disconnect a plug may dislodge the circular scored metal portion in the box wall causing it to come in contact with the hot terminal. The result may lead to an electric shock.
