This was most recently evident during the inspection of the facility located at 76080 Road 338, Madrid, NE 69150, where the employer has inadequately trained employees performing maintenance on equipment throughout the facility. Instances include but are not limited to: i. Lack of classroom or formal presentation followed with an evaluation to validate training. ii. Expedited hands on training without an evaluation process to certify authorized employees.

29 CFR 1910.272(e)(1)(ii): Current employees, and new employees prior to starting work in the grain handling facility were not trained in the specific procedures and safety practices applicable to their job; tasks include but are not limited to: cleaning procedures for grinding equipment, clearing procedures for choked legs, housekeeping procedures, hot work procedures, preventive maintenance procedures, and lockout/tagout procedures. The employer is failing to adequately train employees on required lockout and grain bin entry tasks necessary in the scope of their duties as outlined in this subpart. This was most recently evident during the inspection of the facility located at 76080 Road 338, Madrid, NE 69150, where the employer has inadequately trained employees entering a grain bin with the sweep auger running as part of there process for cleaning out the bins, thus exposing the employees to potential electrical and caught between hazards. Instances include but are not limited to: i. Lack of classroom or formal presentation followed with an evaluation to validate training. ii. Expedited hands on training without an evaluation process to certify authorized employees.

29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked off, or prevented from operating by other means or methods: The employer failed to protect employees from potential electrical and amputation hazards associated with working inside a grain bin with a sweep auger running. This was most recently evident during the inspection of the facility located at 87784 474th Ave, Atkinson, NE 68713, where the employer has employees entering grain bins to clean them out and leaves the sweep auger running to aid the workers in the process. The employer is not having the sweep auger locked out thus exposing the employees to potential electrical and amputation hazards.
