You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at the G Mechanical facility, located at 6635 Singletree Drive, in Columbus, Ohio- The employer did not record teh following workplace injuries and illnesses correctly on the OSHA 300 Log for calendar year 2009. a. For case number 1, while working from a lift, an employee punctured their right hand on ceiling wire while going up in the lift. The recorded description did not provide a date that the injury occurred, an accurate description of how or where the injury occurred, and the case was not classified in columns (G) - (L).

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a. At the facility located 6635 Singletree Drive in Columbus, Ohio, the employer had an exit path in their storage room that was obstructed by a forklift.
