29 CFR 1910.303(b)(1)(iv): Safety of equipment was not determined using considerations for electrical insulation: JOB SITE: 3239 Alfred Avenue, St. Louis, MO 63116, at this work site, located in the maintenance area, workers are allowed to use pedestal buffers to polish lowering devices, and a Power Kraft 180c welding unit, with broken electrical insulation to the electrical power cords. The workers are exposed to an electrical shock or burn hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: JOB SITE: 3239 Alfred Avenue, St. Louis, MO 63116, at this work site, in the welding area outside of the maintenance shop, an electrical panel box had unused openings which were not effectively closed. The workers were exposed to a potential electrocution hazard.
