29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented.                                                                                                               (a) NMS Roofing @ 2540 Hudson St., Denver, CO 80207: On and before 1/25/12, the employer did not ensure that employees utilized a personal fall arrest system. Four employees engaged in roofing activities on a 5 in 12 pitch single-family residential roof. This condition exposed employees to a fall hazard of approximately 11ft.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:    (a) NMS Roofing @ 2540 Hudson St., Denver, CO 80207: On and before 1/25/12, the employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Four employees engaged in roofing activities on a 5 in 12 pitch single-family residential roof were not wearing fall protection. This condition exposed employees to a fall hazard of approximately 11ft.
