One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Tool Room, on or about 1/31/12: Southbend milling machine was not provided with a chuck and tool guard. b) Tool Room, on or about 1/31/12: Bridgeport milling machine was not provided with a chuck and tool guard. c) Tool Room, on or about 1/13/12: Delta Rockwell radial arm drill press was not provided with a chuck and tool guard. d) Tool Room, on or about 1/13/12: Delta #13 single spindle drill press was not provided with a chuck and tool guard. e) Tool Room, on or about 1/13/12: Three Walker Turner single spindle drill presses were not provided chuck and tool guards. f) Tool Room, on or about 1/13/12: Carborundum belt sander was not provided with guarding over the in-running nip points where the belt meets the wheel. g) Glass Cutting Room, on or about 1/13/12: Two (2) Delta single spindle drill presses were not provided with chuck and tool guards. h) Wood Shop, on or about 1/13/12: Delta single spindle drill press was not provided with a chuck and tool guard. i) Plastics Departlment, on or about 1/13/12: Gorton milling machine was not provided with a chuck and tool guard. j) Plastics Department on or about 1/13/12: Bridgeport milling machine was not provided with a chuck and tool guard.
