(a) A Reese Tree Service on or about January 31, 2012, at the address of 1054 E. Center St. Marion, OH: The employer failed to ensure an employee performing tree trimming operations from an elevated vehicle mounted platform, utilized a personal fall arrest system. The employee performing work in this condition was exposed to a potential 35 ft fall to the ground below.

Center St. Marion, OH: The employer failed to ensure an employee was protected from head injuries while performing tree trimming operations from an articulating vehicle mounted platform. The employee was exposed to potential impact injuries caused from falling material and/or equipment.
