29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:     a)  Thiesse Plumbing Inc., did not provide safe means of egress to employees working in a trench with a depth of approximately 9.8 feet.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):      a)  Thiesse Plumbing, Inc., did not protect each employee in its excavation by sloping the excavation or by using appropriate protective system designed in accordance with paragraphs (b) or (c) of 29 CFR 1926.652.  This violation occurred on January 31, 2012, at 931 Ashland Ave, River Forest, Illinois, where employees were working in an excavation that was approximately 9.8 feet deep.       Thiesse Plumbing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA violation number 314965179, citation 1, item 3, issued on November 4, 2010 and became final order on or about November 23, 2010 with respect to a workplace located at Augusta & Monroe, River Forest, IL 60305.     No abatement certification or documentation is required for this item.

Thiesse Plumbing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a) which was contained in OSHA violation number 314965179, citation 1, item 1, issued on November 4, 2010 and became final order on or about November 23, 2010 with respect to a workplace located at Augusta & Monroe, River Forest, IL 60305    No abatement certification or documentation is required for this item.
