29 CFR 1926.451(g)(2):     The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds:    a)  At the jobsite, on or about 2/1/12:  Employees were assebmling and disassembling scaffolding components, including plank, horizontal, and vertical components, at 34 and 58 feet above the lower level, without a fall protection evaluation by a competent person.    Abatement certificaiton must be submitted for this item.

29 CFR 1926.454(a)(2):     Employees have not received training in the correct procedures for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used:    a)  On the jobsite, on or about 2/1/12:  Employees were working on scaffolding and using personal fall protection systems, at 34 and 58 feet above lower levels without training on the use of that equipment in that application.      Abatement certification must be submitted for this item.

29 CFR 1904.4(a):     Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that is recordable and is a new case and meets one or more of the recording criteria.    a)  At the jobsite, on or about 2/1/12: An employee had previously fallen, subsequently became unconcious, was provided medical attention, lost work days, and worked modified duty, without the event being recorded on the OSHA 300 log by the controlling contractor.      Abatement certification must be submitted for this item.
