29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     a) On or about 02/01/12 CSHO observed an employee operating a Bostitch pneumatic nail gun, installing new roofing shingles while not donning any eye and or face protection.

29 CFR 1926.501(b)(11):     29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 02/01/12 CSHO observed three men working on and from an arched roof, installing new roofing shingles with no fall protection systems in place.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) On or about 02/01/12 CSHO observed three men used a 20 foot portable aluminum ladder to access the roof when that ladder did not extend three feet above the landing.
