On the roof of the building where roofing work was performed, there were three extension cords used to provide power to portable tools which did not have ground prongs thereby exposing employees to a contact hazard.

29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, shall be 42 inches (1.1 m)plus or minus 3 inches (8 cm) above the walking/working level. When conditions warrant, the height of the top edge may exceed the 45-inch height,provided the guardrail system meets all other criteria of this paragraph.Note: When employees are using stilts, the top edge height of the top rail, or equivalent member, shall be increased an amount equal tothe height of the stilts. a. On the second level where employees were gaining access to the roof to perform construction tasks, the wire rope top rail was maintained at 27 inches and deflected to 8 inches from the concrete floor thereby exposing employees to a 12 foot fall hazard.

On the south side of the roof where the employer was performing trim work, the warning line was erected 4 1/2 feet from the edge of the roof thereby exposing employees to a 38' foot fall hazard.

In the area where access was set up for employees to gain access to the second floor, the ladder was set up so that employee had to climb through a wire rope guardral from the ladder thereby exposing employees to a fall hazard.
