29 CFR 1926.405(g)(2)(iv):  Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:      (a)  On or about January 31, 2012, and at times prior, employees were not protected from shock and burn hazards from extension cords that had inadequate strain relief.  The extension cords were in use on the Ground Heater at a school under construction located at 25233 E. Glasgow Drive in Aurora, Colorado.
