29 CFR 1910.1030(c)(1)(iv):     The Exposure Control Plan was not reviewed and updated at least annually:        a) Throughout the facility, on or about 2/2/12: The exposure control plan was not reviewed to ensure that proper procedures were implemented and documented including, but not limited to,:         1) using tuberculocidal disinfectants to clean up surfaces contaminated with blood,         2) ensuring that only sharps equipped with engineered sharps injury protection are used,         3) ensuring laundry contaminated with blood is disposed of properly,         4) ensuring hepatitis B vaccinatioins and their titers are provided in a timely manner         5) requiring documantation of employee hepatitis B vaccination status, and         6) documeting proper procedures following an exposure incident.
