29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.     (a) Abdoo Wrecking, LLC on or about February 02, 2012 at the address of 1700 Fostoria Ave. Findlay, OH: The employer failed to ensure employees were protected from head injuries while working below overhead hazards. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment while performing demolition operations.

(a)  Abdoo Wrecking, LLC on or about February 02, 2012 at the address of 1700 Fostoria Ave. Findlay, OH: The employer failed to ensure employees performing demolition work atop air handler #22 on a commercial structure were adequately protected from falls. Employees were exposed to fall up to 13 to the level below.    (b)  Abdoo Wrecking, LLC on or about February 02, 2012 at the address of 1700 Fostoria Ave. Findlay, OH: The employer failed to ensure an employee performing demolition work atop air handler #26 on a commercial structure was adequately protected from falls. The employee was exposed to fall up to 15 to the level below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    (a)  Abdoo Wrecking, LLC on or about February 02, 2012 at the address of 1700 Fostoria Ave. Findlay, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing demolition operations from an elevated level.
