(a) Abdoo Wrecking, LLC on or about February 02, 2012 at the address of 1700 Fostoria Ave. Findlay, OH: The employer failed to ensure employees were protected from head injuries while working below overhead hazards. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment while performing demolition operations.

Employees were exposed to fall up to 13 to the level below. (b) Abdoo Wrecking, LLC on or about February 02, 2012 at the address of 1700 Fostoria Ave. Findlay, OH: The employer failed to ensure an employee performing demolition work atop air handler #26 on a commercial structure was adequately protected from falls. The employee was exposed to fall up to 15 to the level below.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.
