29 CFR 1910.134(c)(2)(i): Where the employer provided respirators for employees to use voluntarily, the employer did not provide the respirator users with the information contained in Appendix D of this section ("Information for Employees Using Respirators When Not Required Under the Standard"):    a) Ronald Mark Associates Inc., Hillside, NJ site - Dry Blend Area: The Employer did not provide employees, voluntarily wearing filtering face-piece respirators, with the information contained in Appendix D of this section (Information for Employees Using Respirators When Not Required Under the Standard).    Ronald Mark Associates Inc., was previously cited for other than serious violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.134(c)(2)(ii) which was contained in OSHA Inspection Number 311914931, Citation #01, Item #001, issued on 09/12/2008 with respect to a workplace located at this same worksite of Hillsdale, NJ.  This citation was not amended and became final order on 10/09/08.       Violation noted on/or about February 9, 2012.
