1926.502(d)(15)    Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.    Jobsite at 1461 S. 12th, Omaha, NE, 68105-Worker was observed stripping shingles off a roof while using a personal fall arrest system that was not anchored to the roof in accordance with the manufacturers design.  The anchorage was installed incorrectly in that it was not secured to both sides of the roof ridge and was not completely attached to the roof deck.  In addition, the employer did not provide evidence that the anchorage, as installed, was capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee who might be exposed to fall hazards which would enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.    Jobsite at 1461 S. 12th, Omaha, NE, 68105-Worker was observed stripping shingles off a roof while wearing a personal fall arrest system that was not properly anchored to the roof.

29 CFR 1926.503(b)(1):     The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record. There was not written certification record containing the name or other identity of the employee trained, the date(s) of the training,and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by anotheremployer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the priortraining was adequate rather than the date of actual training.
