29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.    On or about February 1, 2012 an employee was working from a platform that was supported by a rough terrain forklift that came within 10 feet of a live over-head power line.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about February 1, 2012 an employee was working from a platform that was supported by a rough terrain forklift that was at a height greater than 10 feet with no fall protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      Employees were working form a low sloped commercial roof at a height greater than 6 feet with no fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      Employees were accessing a roof with an extension ladder that did not extend 3 feet past the landing surface nor was it tied off.
