29 CFR 1910.303(g)(2)(i):  Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, covers or by other suitable means:    a) On or about 02-02-12, In the IT room:  An  Oster microwave oven and a  Gold Star refrigerator were connected to a 120 volt AC power via an extension cord.  The extension cord was plugged into a receptacle in a 4 receptacle obox that lacked a cover, exposing live parts.    b) On or about 02-02-12, In the Warehouse: The cover on the pallet wrapper control box was left off exposing electrical parts. Although the disconnect for the pallet wrapper was in the "off" position, there was  no lock or tag on the handle of the disconnect switch to prevent someone from moving the switch to the "on" position, thereby energizing the parts in the control box.    c) On or about 02-02-12, in the Warehouse Entrance Area: An rectangular opening in the wall containing energized conductors at 120 volts-ac with only "wire nuts" covering the live parts lacked a cover.      Abatement certification required.

The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.    a) On or about 02-02-12, In the IT room:  An  Oster microwave oven and a  Gold Star refrigerator were connected to  20 volt AC power via an extension cord.  The plug on the extension cord lacked a ground prong.    Abatement certification required.

29 CFR 1910.37(b)(4):    Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    a. On or about 02-02-12, in the back part of the warehouse: The way to get out of the warehouse to the main hallway on the second floor of the building was not immediately apparent.
