29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which were subjected to heat, might produce flammable or toxic vapors:    10580 S. 147th Street, Omaha, NE 68138 (Blast Bay): The employer is failing to ensure that welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which were subjected to heat, might produce flammable or toxic vapors. This violation was most recently observed at a jobsite located at 10580 S. 147th Street, Omaha, NE 68138.An employee was cutting on a 55-gallon drum which previously held lacquer thinner (a DOT Class 3 Flammable Liquid) in order to complete work on a coupler for a bulk trailer.  The drum had not been cleaned or purged when the employee began cutting on it. The drum exploded; causing significant injuries to the employee.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
