29 CFR 1926.754(e)(3)(i): Covers for roof and floor openings were not capable of supporting, without failure, twice the weight of the employees, equipment and materials that may be imposed on the cover at any one time:    a) Employees were exposed to the hazard of falling approximately 15 feet through skylights while installing metal roof decking and related components.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Employees were exposed to the hazard of falling approximately 18 feet where a portable extension ladder did not extend at least 3 feet above the roof line being used as a landing surface along the rear of the building.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
