1904.29(b)(1) A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completely in detail as required by the regulation; a. Entire Facility - On or February 7, 2012, the employer's OSHA 300 Log forms were missing information on column F. The number of entries per year that were missing information were the following; 1. Seven entries in the OSHA 300 Log form for the year 2010 were missing information such as the object/substance that directly injured or made person ill. 2. Nine entries in the OSHA 300 Log form for the year 2011 were missing information such as the object/substance that directly injured or made person ill. 3. Three entries in the OSHA 300 Log form for the year 2012 were missing information such as the object/substance that directly injured or made person ill.
