29 CFR 1910.132(d)(1)(i):     When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard of fall:   a) On or about 02/09/12 at the bulk truck loading area; bulk over the road trucks are filled with sand.  Truck driver opens two of the hatch compartments and positions the truck over the conveyor spout.  A site employee climbs on top of the truck to position the fill spout into the open hatch and climbs down from the truck.  This operation is repeated for the next compartment and two compartments are filled on each truck.  The height of the work platform is 13 feet, this climbing onto the truck is performed outdoors and no fall protection is provided to protect employee against falling.    Abatement Documentation
