29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     In the Grinding Room at the Blender Machine, an employee was exposed to a fall hazard from an inadequate guardrail system on the open-sided floor of the working platform from a top rail chain having a sag of 12 inches and no midrail.

29 CFR 1910.147(c)(6)(i):     The employer shall conduct a periodic inspection of the energy control procedure at least  annually to ensure that the procedure and the requirements of this standard are being followed.    Periodic inspections of the Company's energy control procedures had not been conducted on at least an annual basis.

29 CFR 1910.147(d)(6):     Verification of isolation.  Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.    On or about January 24, 2012, in the Grinding Room, an employee was tearing down the Hydroflaker and did not verify the isolation after affixing the lockout device on the electrical energy source by trying to start the machine with the on button.

29 CFR 1910.332(b)(1):     Practices addressed in this standard. Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.    Employees performing energized electrical work were inadequately trained on safety-related work practices in that affected employes were unfamiliar with when and what PPE to wear when performing energized electrical work.

Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6.    Insultated rubber gloves used by employees performing energized electrical work had not been inspected based on the test voltage and interval between tests.
