29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     In the Grinding Room at the Blender Machine, an employee was exposed to a fall hazard from an inadequate guardrail system on the open-sided floor of the working platform from a top rail chain having a sag of 12 inches and no midrail.

29 CFR 1910.147(d)(6):     Verification of isolation.  Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.    On or about January 24, 2012, in the Grinding Room, an employee was tearing down the Hydroflaker and did not verify the isolation after affixing the lockout device on the electrical energy source by trying to start the machine with the on button.

Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments. 29 CFR 1910.178(l)(4)(iii):     An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.    Operators of powered industrial trucks, sit-down forklift and power pallet truck, had not received an evaluation of their operator performance once every three years.
