29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Centurion Roofing LLC at 3303 N. York St., Denver, CO 80204: On or before November 30, 2011, the employer did not ensure the employees were protected from injury, in that employees engaged in construction activities on a roof with a fall hazard of approximately 20-25 feet without the use of a fall protection system.
