29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. (a) Coil straighteners exposed press operators and other employees to a tripping and cut hazard in-that the coils were not identified or cordoned off to prevent employees from inadvertently walking into the coils being feed into presses.

29 CFR 1910.305(b)(1)(i): Openings through which conductors enter boxes, cabinets, or fittings were not effectively closed. (a) Electrical boxes located on press number 3 (Niagra 90 ton press) did not have the openings through which electrical wiring entered the box(es) effectively closed. The flexible metalic cable through which the conductors were run, was broken away from the connection fitting, exposing the conductors. This exposed employees to an electrical shock hazard.
