29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means.    Location: 389 8th Ave., New York, NY.    a) Employer failed to ensure that the electrical panels at the jobsite were protected against accidental contact as they were hung at a height of approximately 5 ft from the floor.  Employees were exposed to an electrical shock hazard; on or about 1/26/12.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: 389 8th Ave., New York, NY.    a) Employer failed to ensure that an employee was protected from falling while installing the building's temporary stair components.  Employee was exposed to a potential fall hazard of up to 9 ft in height; on or about 1/26/12.
