29 CFR 1910.157(g)(2):     The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    Industrial Paint & Strip West, LLC, located at 1000 Commerce Ct., Waynesfield, OH: On or about February 7, 2012, the employer failed to provide employees with an educational program upon initial employment and at least annually thereafter on the use of portable fire extinguishers and the hazards associated with incipient stage fire fighting, exposing employees to hazards associated with fire suppression and operation of fire suppression equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.178(q)(7):     Industrial trucks were not examined before being placed in service, or were placed in service if the examination showed conditions adversely affecting the safety of the vehicle:    Industrial Paint & Strip West, LLC, located at 1000 Commerce Ct., Waynesfield, OH: On or about February 7, 2012, the employer did not ensure that daily examinations of the forklifts such as, but not limited to, Toyota # 1 (Model 8FGCU25, Serial # 27058), Toyota # 3 (Model 8FGCU25, Serial # 27074), and Yale (Model GLC030AFNUAV084, Serial # A809N11177X) were conducted before being put into service. Employees were exposed to struck-by and caught in between hazards.    No additional abatement information is needed for this item.
