29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a)Rail car loading area: On or about February 7, 2012 an employee working at 156 high on the upper deck of a rail car was tied off at his feet with a personal fall arrest system that included a 6 foot lanyard which if put in to service would splay out approximately 18 feet thus allowing the employee to hit the ground below prior to the fall arrest system coming in to play. Note: Abatement certification is required for this item.

29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: Maintenance Shop: On or about February 7, 2012 sling hooks being used to lift animal carcasses for skinning were not inspected and were found to have faulty safety latches and small amount of spreading. 29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): Maintenance Shop mezzanine: On or about February 7, 2012 a fluorescent light fixture hung approximately 61.5 inches from the ground and had no guarding to protect the bulbs from being broken and exposing employees to live electrical parts and broken glass.
