29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a) On or about February 2, 2012 and at times prior thereto, employees were exposed to an amputation hazard while operating the equipment referred to as the tree stake pointer which had an unguarded saw used to cut the stakes to length.    b) On or about February 2, 2012 and at times prior thereto, employees were exposed to an amputation hazard while operating the large pointing equipment which had and unguarded blade.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains were not enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.    a) On or about February 2, 2012 and at times prior thereto, employees were exposed to amputation while working around the unguarded chain and sprocket on the log transfer table for the large peeler.    b) On or about February 2, 2012 and at times prior thereto, employees were exposed to an amputation while using the stairs to access the work station due to the unguarded drive chain and sprocket for the log tranfer table.    c) On or about February 2, 2012 and at times prior thereto, employees were exposed to an amputation while working near the unguarded drive chain and sprocket on the green chain.    d) On or about February 2, 2012 and at times prior thereto, employees were exposed to an amputation hazard while working near the unguarded chain and sprocket on the tree stake dowl machine.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt was not fully enclosed in accordance with paragraphs (m) and (o) of this section.    a) On or about February 2, 2012 and at times prior thereto, employees exposed to a caught in hazard while working around and near the unguarded belt and pulley on the green chain.    b) On or about February 2, 2012 and at times prior thereto, employees exposed to a caught in hazard while working around and near the unguarded pulleys on the equipment used to cut the tree stakes to length.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and they did project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.    a) On or about February 2, 2012 and at times prior thereto,  employees were exposed to the unguarded shaft end located on the red roller near the operator of the large peeler.      b) On or about February 2, 2012 and at times prior thereto, employees were exposed to the unguarded shaft end on the point stake machine.      c) On or about February 2, 2012 and at times prior thereto, employees were exposed to the unguarded shaft ends on the small peeler.
