29 CFR 1910.137(b)(2)(ii): Insulating equipment was not inspected for damage before each day's use and immediately following any incident that could reasonably be suspected of having caused damage: On or about February 8, 2012, an employee was exposed to an electrical shock as the voltage rated gloves he was wearing were not inspected prior to use. In accordance with 29 CFR 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.137(b)(2)(iii)(A): Insulating equipment with any of the following defects shall not be used; a hole, tears, puncture or cut. On or about February 8, 2012, an employee was exposed to an electrical shock while wearing voltage rated gloves that were damaged with a hole, tear, puncture or cut. In accordance with 29 CFR 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
