29 CFR 1926.451(b)(1): Each platform on all working levels was not fully planked or decked between front uprights and the guardrail supports: a) Employees were exposed to fall hazards of approximately 16.4 feet while working on a scaffold platform which was not fully decked. Illinois Home Improvement Corporation dba Best Brick Masters was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1) which was contained in OSHA violation number 312594500, citation 1, item 1b, issued on May 22, 2009 and became final order on or about June 17, 2009 with respect to a workplace located at 815 W. Weed St., Chicago, IL 60642. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Weed St., Chicago, IL 60642. Weed St., Chicago, IL 60642, and OSHA violation number 314075631, citation 2, item 1, issued on March 15, 2010 and became final order on or about April 6, 2010 with respect to a workplace located at 1516 Edgewater, Chicago, IL 60660. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
