29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: On or about February 09, 2012 and at times prior thereto, employees were exposed to a fall of approximately 12 feet to the ground below while installing shingles from a near vertical roof on scaffolding that was not fully planked or decked.
