29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous:    (a) Near fire hydrant 27 and next to building 939 A, on or about February 8, 2012, there was no grounding pin on the plug to a 110 VAC, MultiEquip submersible pump, model number 2005 T.  The pump was powered from a 3000 Watt, Honda portable generator.  The pump was used remove water from a trench that contained a water pipe for fire hydrant 27.  Three wires were observed in the cord to the pump after the plug was cut off the cord.
